Connected Care Program (Sick Kids)

Connected Care provides support to family caregivers and community health care providers who are caring for children with medical complexity and who depend on the use of medical technologies.

It delivers three main services: education, virtual outreach and bridged transitions.

Connected Care also offers programming to support home and community providers caring for children with medical complexity and technology dependence. Connected Care offers full and half-day competency-based education modules, access to 24/7 nurse lead virtual-consultative services and a community of practice.

Connected Care is a Ministry of Health and Long Term Care (MOHLTC) funded program lead by executive sponsors at both The Hospital for Sick Children (SickKids) and the Toronto Central Local Health Integration Network (TC-LHIN)
